One file per pet
Species, breed, date of birth, weight over time, vaccinations with batch numbers, medications, allergies, vet contact, insurance if any.
For the vet
Print the last year. Bring it. It saves the receptionist ten minutes of searching for the boosters they gave in 2024.
For the boarder
Same document; different reader. Boarding kennels care about vaccinations and behavioural notes — both live in the same file.

Insurance claims?
The vet visits log doubles as the claim record. Print, submit.
Microchip?
Add the chip number and registration contact in the file's header. It's the thing you'll wish you'd noted.
What about training?
Add training notes in the same file, or in a separate one. Whatever fits.
